This was absolutely intentional. Watch this in slow motion and you'll see that the woman behind her clearly takes her top and opens it from the back, while VERY intentionally spreading her hands apart as to ensure it would be fully opened in the back and fall off. She reaches right for it, and pulls it down and out. And look at her expression as she undoes her top and you'll see she's not surprised at all and even has a suspiciously delayed reaction. This may have been sabotage, but it was not an accident.
