Araragi and Karen's relationship is probably one of the most ambiguous things in that series. All sorts of things are implied about it and Hitagi not only seems to know about it, but seems to be okay with it too. I am convinced that they banged one night and all the other characters know but they decided to sweep it under the rug for whatever reason.
